Le SCAD est une application de télémédecine permettant un suivi clinique et éducatif à domicile de patients atteints de maladies chroniques.
C’est un système interactif mettant en relation le patient et l’équipe de suivi via un algorithme adapté à sa pathologie.
Des questions quotidiennes, personnalisées, sont posées au patient sur les symptômes, les signes vitaux, l’alimentation, l’activité physique.
Les données sont ensuite traitées par un algorithme qui, en fonction des réponses, adaptera les questions et les conseils. L’algorithme peut aussi générer des alarmes selon
les réponses saisies par le patient.
L’équipe de suivi reçoit les éléments transmis, les analyse et réagit, si nécessaire, par le biais de l’application voire par communication téléphonique avec le patient en
cas d’alarme.
Le patient suit et renseigne les différentes demandes devenant ainsi peu à peu acteur de sa prise en charge. Il peut également accéder, ainsi que son entourage, à des
informations sur sa maladie chronique.
Une fonction correspondance facilite les interactions entre le patient et l’équipe de prise en charge.
L’application SCAD n’est pas un système d’urgence.
